Nominees
If you purchased or otherwise acquired publicly traded common stock of Olaplex Holdings, Inc. from September 29, 2021 through November 12, 2021, inclusive, for the beneficial interest of persons or entities other than yourself, you must within ten (10) calendar days of receiving the Postcard Notice:
- Provide a list of the names, addresses and emails of all such beneficial owners to the Claims Administrator; or
- Request from the Claims Administrator sufficient copies of the Postcard Notice to forward to all such beneficial owners and mail those Postcards yourself within ten (10) calendar days after receiving them. Once sent, send a statement to the Claims Administrator confirming the mailing was made. Please also provide email addresses to the Claims Administrator, to the extent available.
If you are providing a list of names, addresses and emails to the Claims Administrator:
- Compile a list of names, last known addresses and email addresses of the beneficial owners that purchased or otherwise acquired publicly traded common stock of Olaplex Holdings, Inc. during the period September 29, 2021 through and including November 12, 2021.
- Prepare the list in Microsoft Excel format using the template for submitting mailings data here. Failure to use this template may delay the submission of the mailing data.

Expense Reimbursement:
Upon full and timely compliance with the above directions, Nominees may seek reimbursement of their reasonable expenses actually incurred in complying of up to $0.05 per name/address provided and up to $0.05 plus postage at the Claims Administrator’s rate for bulk mailings, by providing the Claims Administrator with proper documentation supporting the expenses for which reimbursement is sought. Properly documented expenses incurred by Nominees in compliance with these instructions shall be paid from the Settlement Fund, with any unresolved disputes as to the reasonableness or documentation of expenses subject to review by the Court. Please submit your invoice within one month of completing the mailing or providing your file and send it to Info@OlaplexSecuritiesSettlement.com. Be sure to include the case name and “Reimbursement” in the subject line of your email.
